Simplifying and adapting antiretroviral treatment in resource-poor settings: a necessary step to scaling-up.

چکیده
Access to antiretroviral (ARV) therapy for patients in developing countries has become an increasing global public health and political concern in recent years. Both donor governments and those of high-burden countries agree that treating AIDS is a major priority. Efforts are underway in many countries to increase the numbers receiving ARV treatment, and the level of international funds committed to assist this effort has increased significantly, albeit insufficiently. In 2002, this led the World Health Organization (WHO) to launch an ambitious ‘335’ plan to place three million people on ARV by 2005 [1].

Simplification of antiretroviral therapy: a necessary step in the public health response to HIV/AIDS in resource-limited settings.
The global scale-up of antiretroviral therapy (ART) over the past decade represents one of the great public health and human rights achievements of recent times. Moving from an individualized treatment approach to a simplified and standardized public health approach has been critical to ART scale-up, simplifying both prescribing practices and supply chain management. In terms of the latter, the...
متن کاملScaling up antiretroviral treatment in resource-poor settings.
Anthony Harries and colleagues (June 3, p 1870) do well to explore the tensions between technical and public-health approaches to the HIV/ AIDS epidemic in Malawi. However, they miss opportunities to discuss several fundamental issues. Important lessons could be learned from neighbouring countries. In Mozambique, the antiretroviral treatment (ART) scale-up programme is very similar to that of M...
متن کاملTackling HIV in resource poor countries.
Despite recent proposals to expand access to antiretroviral treatment for people with HIV in resource poor settings, debate continues about the cost effectiveness of prevention and treatment strategies. This debate delays the urgent action needed to implement a comprehensive global AIDS strategy. To underscore the need to link prevention with treatment, we review the effect of HIV on economic a...

Methods: This was a cross - sectional study, in which 200 pregnant women of reproductive age were selected using multist...
متن کاملImproving the evidence base of Markov models used to estimate the costs of scaling up antiretroviral programmes in resource-limited settings
BACKGROUND Despite concerns about affordability and sustainability, many models of the lifetime costs of antiretroviral therapy (ART) used in resource limited settings are based on data from small research cohorts, together with pragmatic assumptions about life-expectancy. This paper revisits these modelling assumptions in order to provide input to future attempts to model the lifetime costs an...
